Michael Madsen, who was found unresponsive in his Malibu home on July 3, died from heart failure, with heart disease and alcoholism as contributing factors, his rep said.

"The doctor's report is accurate that he died from heart failure," said Smith, referring to a statement that Madsen's cardiologist made to NBC4 Los Angeles. He added that "heart disease and alcoholism" were named as "contributing factors."
